Toaster Oven/Broilers Recalled by Haier America Due to Burn or Electrical Shock Hazard

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Toaster Oven/Broilers

Units: About 106,000

Importer: Haier America Trading L.L.C., of New York, N.Y.

Manufacturer: Lun Dar Electric Ind. Ltd., of Taiwan

Hazard: Electrical connections in the toaster oven/broilers can become loose, posing electrical shock and burn hazards.

Incidents/Injuries: Haier America has received two reports of minor burns and one report of a minor electrical shock to consumers, and one report of minor property damage.

Description: This recall involves toaster oven/broilers with model number RTO1400SS. The units are stainless steel and black plastic. “Haier” is printed on the front and the model number is printed on a label on the back of the toaster oven/broilers.

Sold at: Mass merchandisers and specialty retailers nationwide from September 2006 through January 2009 for approximately $20 to $40.

Manufactured in: China

Remedy: Consumers should immediately stop using the recalled toaster oven/broilers and contact Haier America to receive a free replacement toaster oven/broiler.

The following product safety recall was voluntarily conducted by the firm in cooperation with the CPSC. Consumers should stop using the product immediately unless otherwise instructed.

Hazard: The handrails and posts on the playground contain high levels of lead paint. Lead can be toxic if ingested by young children and can cause adverse health effects.

Incidents/Injuries: None reported.

Description: The recall involves the Tot Town Fun Center, Tot Town Castle Fun Center, Tea Cup Merry Go Round, 8’ Web Climber, Rope Wall, Four Panel Circular Aztec Climber and Three Panel Aztec Climber.

Specialized Bicycle Components Recalls Bicycles Due to Serious Fall Hazard

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Specialized 2009 Model Bicycles

Units: About 14,000

Importer: Specialized Bicycle Components Inc., of Morgan Hill, Calif.

Hazard: The handlebars can break, posing a fall hazard to the rider.

Incidents/Injuries: Specialized has received two reports of the handlebar breaking during use, one of which caused a rider to suffer head trauma which resulted in unconsciousness and a puncture wound to the arm.

Description: This recall involves HL handlebars on the following 2009 Specialized bicycles: Cross Trail Comp, Myka HT Comp, Myka HT Elite, Rockhopper, Rockhopper Comp and Rockhopper Comp 29. The model number of the HL handlebar is ALR-13 NTFOV and is located underneath the right hand grip of the handlebar.

Sold by: Authorized dealers nationwide from June 2008 through February 2009 for between $640 and $1,050.

Fitness Balls Recalled by EB Brands Due to Fall Hazard; New Assembly Instructions Provided

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product.

Name of Product: Bally Total Fitness, Everlast, Valeo and Body Fit Fitness Balls

Units: About 3 million

Manufacturer: EB Brands, of Yonkers, New York

Hazard: An overinflated fitness ball can unexpectedly burst while in use, causing the user to fall to the floor.

Incidents/Injuries: EB Brands has received 47 reports of fitness balls unexpectedly bursting, including reports of a fracture, and multiple bruises.

Description: This recall involves rubber fitness balls sold in 55-, 65- and 75-cm diameter sizes. The fitness balls were sold with a pump and inflation instructions. The fitness balls were sold in various colors with the Bally Total Fitness, Everlast or Valeo logo printed on the ball.

Sold at: Department stores and fitness retailers nationwide from May 2000 to February 2009 for between $15 and $30.

Toy Maracas Recalled by Tupperware U.S. Due to Choking and Suffocation Hazards

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Shape-O Toy Maracas

Units: About 44,000

Importer: Tupperware U.S. Inc., of Orlando, Fla.

Hazard: The maracas can break and expose small parts, posing a choking hazard to young children. In addition, the handle poses a suffocation hazard to young children.

Incidents/Injuries: Tupperware has received two reports of maracas breaking. No injuries have been reported.

Description: This recall involves blue and red toy maracas with yellow handles. The maracas were used by Tupperware representatives during sales presentations. Cut-outs in the shape of stars, squares and other geometric figures are located inside the maraca.

Sold by: Tupperware’s Holiday 2007 and Spring 2008 product catalogs from September 2007 through April 2008 for between $5.50 and $11.

Remedy: Consumers and Tupperware sales consultants should immediately stop using the maracas and return them to Tupperware for a refund or gift certificate. Maraca return forms can be accessed at www.tupperware.com

Bush Hog Off-Road Utility Vehicles Recalled Due to Loss of Speed Control; Cash Incentive Offered

The following product safety recall was voluntarily conducted by the firm in cooperation with the CPSC. Consumers should stop using the product immediately unless otherwise instructed.

Name of Product: Bush Hog Off-Road Utility Vehicles

Units: About 850 units

Manufacturer: Bush Hog LLC, of Selma, Ala.

Hazard: The utility vehicle’s throttle cable can lock in freezing temperatures. This can cause the engine not to return to idle when the driver takes his or her foot off the accelerator pedal, posing a risk of loss of vehicle control and injury to the driver and passenger.

Incidents/Injuries: Bush Hog has received one report of an incident involving a frozen throttle cable on a utility vehicle. No injuries have been reported.

Description: The recall includes the Bush Hog Models TH4400 (Trail Hand) and 4430 4X4 Off-Road Utility Vehicles. “Bush Hog” is printed on the utility vehicle’s cargo bed tail gate and on each side of the cargo bed. Model “TH4400”or “4430” is printed on each side of the hood. The hood color is red, green, or mossy oak.

Sold by: Bush Hog dealerships nationwide from April 2008 to December 2008 for between $8,000 and $10,000.

Remedy: Consumers should immediately stop using the recalled off-road utility vehicles and contact a Bush Hog dealer to schedule a free inspection and repair. All registered owners have been notified about this recall by mail. Bush Hog is issuing a $50 incentive check for retail customers that bring their machines in for the throttle cable replacement.

Senseo One-Cup Coffeemakers Recalled by Philips Consumer Lifestyle Due to Burn Hazard

Name of Product: Philips Senseo One-Cup Coffeemakers

Units: About 155,000

Distributor: Philips Consumer Lifestyle, of Stamford, Conn.

Hazard: An electrical fault and the build-up of calcium from hard or medium water can cause an obstruction in the coffeemaker. If this happens, the boiler can burst, posing a burn hazard to consumers.

Incidents/Injuries: None reported in the United States. The firm has received 17 reports of incidents in Europe, including six reports of minor personal injury involving first degree burns to the hands, arms and abdomen.

Description: This recall involves Senseo one-cup coffeemakers with model numbers HD 7810, HD 7811, HD 7815, HD 7820, HD 7832, and HD 7890. Model numbers are located on the bottom of the coffeemaker. Date codes are printed on the bottom of the coffeemaker. Coffeemakers made in China have date codes 0727 through 0847; coffeemakers made in Poland have date codes 0627 through 0847.

Sold at: Wal-Mart, Target and Safeway stores nationwide and online at Amazon.com between July 2006 through March 2009 for between $60 and $140.

Manufactured in: China and Poland

Mega Brands America To Pay $1.1 Million Civil Penalty For Reporting Violations With Popular Magnetic Building Sets

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC) announced today that Mega Brands America Inc., of Livingston, N.J., formerly Rose Art Industries Inc., has agreed (PDF) to pay a $1.1 million civil penalty. The penalty, which the Commission has provisionally accepted, settles allegations that Mega Brands America and Rose Art failed to provide the government with timely information about dangers to children with Magnetix magnetic building sets, as required under federal law.

In December of 2005, Rose Art filed an “initial report” with CPSC that a 22-month-old child from Washington state had died, due to ingesting multiple magnets that fell out of pieces from a Magnetix set. The report contained no other product or incident information and Rose Art attributed the magnets falling out to unusually abusive play by the toddler’s older siblings. On February 1, 2006, Rose Art submitted a Full Report which again lacked incident and product information. Rose Art stated that it did not retain any complaint or incident records. On March 31, 2006, Rose Art voluntarily recalled nearly 4 million Magnetix sets for users under the age of 6.

After discovering documents which led CPSC staff to believe Rose Art had compiled incident information, a subpoena was issued to the firm (which had been renamed Mega Brands America and was under new ownership and control) to obtain product and incident information. CPSC learned through the subpoena that at the time Rose Art filed its “initial report” in December 2005, it had received over 1,100 consumer complaints that magnets had fallen out of plastic pieces from dozens of different Magnetix models. Additionally, the subpoena revealed that Rose Art had received at least one report of an injury due to magnet ingestion, prior to the toddler’s death in Washington state.

By the time Rose Art agreed to the recall of Magnetix in March 2006, the firm had received more than 1,500 complaints of magnets falling out of plastic pieces in more than 65 different models of Magnetix. In April 2007, Mega Brands America expanded the recall of Magnetix sets for users of any age, after more than 25 children suffered intestinal injuries that required surgery to remove the magnets.

Federal law requires firms to report to CPSC immediately (within 24 hours) after obtaining information reasonably supporting the conclusion that a product contains a defect which could create a substantial product hazard, creates an unreasonable risk of serious injury or death, or violates any consumer product safety rule, or any other rule, regulation, standard, or ban enforced by CPSC.

In agreeing to settle this matter, Mega Brands America and its parent, Mega Brands Inc., of Montreal, Canada contend that 1) Mega Brands Inc. did not know of the Magnetix defect at the time it acquired Rose Art and 2) Rose Art’s prior owners never advised Mega Brands Inc. of the problems of associated with Magnetix.

CPSC strongly encourages consumers to check (PDF) to see if they have any of the recalled building sets and return them to Mega Brands for a free replacement toy – potentially millions of recalled units remain in homes today and accessible to young children.

Incidents/Injuries: Ross Stores has received one report of a consumer who suffered shoulder pain after her chair collapsed.

Description: The folding wooden patio chairs are made of eucalyptus. SKU 400037791757 and manufacturer style number KTOC-1638-ROS are printed on a hangtag attached to the chair.

Sold at: Ross Stores nationwide in February 2009 for about $50.

Sold exclusively at: Wal-Mart stores nationwide from June 2008 through December 2008 for about $13.
