Name of Product: Anthropologie Tin Candles
Units: About 225
Importer: Candela Group, of New York, N.Y.
Hazard: The candle flames could flare up out of the tin container during the burning of the last half inch of wax, posing a fire and burn hazard.
Incidents/Injuries: None reported.
Description: This recall involves Anthropologie tin candles. The candles were sold in the following four fragrances; Amber Woods – Model #14987044, Black Lilac – Model #14987093, Figue Tree – Model #14987085 and Red Currant – Model #14987051. Candles sold after October 11, 2008 are not affected by this recall.
Sold at: Anthropologie stores nationwide from September 2008 through October 2008 about $20.

Remedy: Consumers should immediately stop using the recalled tin candles and call Candela for a self-addressed, postage paid envelope in which to return the candle for a full refund.

Serta International Recalls Mattresses Due to Violation of Federal Mattress Flammability Standard
WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: Serta Zipper-Covered Foam Core Mattresses
Units: About 6,100
Manufacturer: Serta International Mattress Co., of Hoffman Estates, Ill.
Hazard: The mattresses fail to meet the mandatory federal open flame standard for mattresses, posing a fire hazard to consumers.
Incidents/Injuries: None reported.
Description: The recalled mattresses have a zipper cover over a foam core. The mattresses were sold in twin, full, queen, king and California king sizes. They were manufactured between November 30, 2007 and May 20, 2008. The mattress name, date of manufacture, model, and serial numbers are located on label attached to the mattress. Only specific serial numbers are included in the recall. The recalled mattresses are listed in the chart below.

Name of Product: “Colonial” Folding Game Chairs
Units: About 1,700
Importer: Brunswick Bowling & Billiards Corp., of Lake Forest, Ill.
Hazard: The retaining washers on the legs can loosen, causing the chair to become unstable. This poses a fall hazard to consumers.
Incidents/Injuries: Brunswick has received four reports of loose chair leg hardware. No injuries have been reported.
Description: This recall involves the “Colonial” model, folding wooden chairs for use with game tables. They were sold in sets of two. The chairs have a black seat cushion and either a cherry or chestnut finish.
Sold by: Authorized Brunswick dealers nationwide from March 2007 through March 2008 for about $400 per set.

Remedy: Consumers should immediately stop using the folding game chairs and contact the dealer where the chairs were purchased to schedule a free repair.

Viessmann U.S. Recalls Boilers Due to Fire Hazard
WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: Vitodens 200 Boilers
Units: About 4,600
Manufacturer: Viessmann Manufacturing Co. Inc., of Canada
Hazard: The boiler can leak gas, posing a fire hazard to consumers.
Incidents/Injuries: None reported.
Description: The recall involves the Vitodens 200 boiler. The gas-fired boilers are white, wall-mounted and have “Viessmann” and “Vitodens 200” printed on the exterior in silver letters. The model and serial numbers included in the recall are listed below. The model number is located on the silver rating plate on the side of the boiler. Contact your certified HVAC contractor to open the boiler and locate the serial number on a white label on the side of the boiler, or under the bottom support panel. Vitodens 100 boilers are not included in this recall.

IKEA Recalls to Repair Chest of Drawers Due to Laceration Hazard

Name of Product: �KVIBY� Chests
Units: About 5,000 (an additional 26,000 were sold outside of the U.S.)
Manufacturer: IKEA Home Furnishings, of Conshohocken, Pa.
Hazard: The glass drawer knobs on the chest can break either during assembly or in use, posing a laceration hazard to consumers.
Incidents/Injuries: IKEA has received three reports of laceration injuries requiring medical attention. An additional 11 injuries occurred outside of the U.S.
Description: The recalled KVIBY Chest is white with 4 drawers and 6 glass knobs. The recall includes chests with article number 201-080-90 and a date stamp of 0817 (first two digits indicate the year; the second two digits indicate the week) or earlier. The article number and date stamp are located on a label attached to the underside of the chest. KVIBY chests with a date stamp of 0818 and later have improved replacement knobs and screws and are not included in the recall.
Sold at: IKEA stores nationwide from August 2007 through July 2008 for about $300.
Manufactured in: Denmark
Remedy: Consumers should protect their hands and carefully remove the glass knobs. Contact IKEA to receive free replacement knobs and screws by mail.

- “Madison DLP” Model 060176: The product consists of three glass shelves with dark wood grain finish frame components, black legs, and a black fiberboard back panel.
- “Madison 3000” Model 060090: The product consists of three glass shelves with dark wood grain finish frame components, black legs, and a metal mesh back panel.

Name of Product: SmartSpace™ Papasan Chairs

Hazard: The chairs were sold without a warning label instructing consumers to lock the chair’s legs before sitting. This poses a fall hazard to consumers.
Incidents/Injuries: Jo-Ann has received five reports of chairs collapsing when the legs were not locked. No injuries have been reported.
Description: This recall involves the SmartSpace™ Papasan chair. The silver metal frame chair has a circular fabric seat that sits on a 10-inch-tall base. The fabric seat used with the papasan chairs were sold in purple, pacific, lime yellow and gray. A hangtag on the chair reads “Smartspace Papasan Chair – 1 piece.”
Sold at: Jo-Ann Fabric and Craft stores nationwide from June 2008 through September 2008 for about $25.

Hazard: The fryers can overheat, posing a burn hazard.
Incidents/Injuries: No injuries have been reported.
Description: The recalled fryers include QVC Cook Essentials 3 Liter Stainless Steel Deep Fryers with three baskets. The fryers were sold as item number K14942. Model number F1053Q is printed on a sticker on the bottom of the fryers.
Sold through: QVC’s televised shopping programs and its Web site from April 2008 through May 2008 for about $60 including shipping costs.

CPSC Issues Warning: Dangers at Home in the Aftermath of Hurricane Ike
Carbon Monoxide from Generators Can Kill in Minutes
WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC) warns consumers on the Gulf Coast to protect themselves and their families against dangers in the aftermath of Hurricane Ike.
Portable gasoline generators quickly produce high levels of poisonous carbon monoxide (CO) and should never be used indoors, including inside a home, basement, shed or garage, even if doors or windows are open. CO is an invisible killer. It’s a colorless, odorless, poisonous gas. CO from a generator used indoors can kill you and your family in minutes.
“Heeding this warning can mean the difference between life and death,” said CPSC Acting Chairman Nancy Nord. “Consumers should use portable generators outside only, far from windows, doors, and vents to their homes.”
CPSC issued these additional safety tips for consumers in the aftermath of Hurricane Ike:
- Read the label on the generator and the owner’s manual, and follow the instructions.
- Install CO alarms with battery backup in the home outside each sleeping area.
- Get to fresh air immediately if you start to feel sick, weak or dizzy. CO poisoning from exposure to generator exhaust can quickly lead to incapacitation and death.
- Never use charcoal indoors. Burning charcoal in an enclosed space can produce lethal levels of carbon monoxide.
- Use caution when burning candles. Use flashlights instead. If you must use candles, do not burn them on or near anything that can catch fire. Never leave burning candles unattended. Extinguish candles when you leave the room.
- Look for signs that your appliances have gotten wet. Discard electrical or gas appliances that have been wet because they pose electric shock and fire hazards.
- Before using your appliances, have a professional or your gas or electric company evaluate your home and replace all gas control valves, electrical wiring, circuit breakers, and fuses that have been under water.

PB Heat Recalls Gas Boilers Due to Carbon Monoxide Hazard
WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: Gas Boilers
Units: About 1,200
Manufacturer: PB Heat LLC, of Bally, Pa.
Hazard: The pressure switch could have an internal defect that could fail to shut down the boilers under vent or air inlet blockage. This switch defect could allow the boilers to produce excessive levels of carbon monoxide, posing a risk of carbon monoxide poisoning to consumers.
Incidents/Injuries: None reported.
Description: This recall involves the DE and PSC II Gas Fired Cast Iron Boilers with the Endura Plastics Model ES2035 pressure switch used for home heating and domestic hot water heating. The boilers were available in either natural or LP gas. The boiler rating label is located on the top jacket panel on Series PSCII models and the left side jacket panel on the Series DE models. The label includes the model and serial number. The following model names and serial numbers are included in the recall:
